<strong>Job Title:</strong> Telephonic LPN – Care Coordination (Full remote)<br><br><strong>Location:</strong> Remote (with occasional reimbursed field visits in New Hampshire)<br><br><strong>Job Type:</strong> Temp-to-Perm<br><br><strong>Department:</strong> Care Management / Utilization Support<br><br><strong>Pay Rate:</strong> $34/hour<br><br>Position Summary<br><br>A managed care organization serving New Hampshire Medicaid members is seeking a Licensed Practical Nurse (LPN) to join its Care Management team in a telephonic role. This position begins as a <strong>temporary assignment with the opportunity to transition to permanent employment</strong> based on performance and organizational needs. The LPN will support care coordination, admissions, and member engagement primarily by phone, with occasional in-person visits to skilled nursing facilities (SNFs) or members’ homes as needed (mileage reimbursed). The ideal candidate will have experience in long-term care, post-acute intake, or transitional care coordination and be comfortable working remotely in a structured, fast-paced environment.<br><br>This is the second of two openings. You must live within 30 minutes of Manchester.<br><br>Key Responsibilities<br><br><ul><li>Conduct telephonic outreach to members, families, providers, and facilities to coordinate care, admissions, and services. </li><li>Support transitions of care for members entering or discharging from long-term care, SNF, or rehabilitation settings. </li><li>Gather and review clinical documentation, assessments, and eligibility information for RN or physician review. </li><li>Serve as a liaison between the health plan, facility admissions teams, discharge planners, and providers to ensure timely and efficient placements. </li><li>Assist with prior authorization requests, documentation preparation, and Medicaid eligibility verification. </li><li>Educate members and families on benefits, services, and expectations during transitions of care. </li><li>Document all interactions, activities, and clinical notes accurately in the electronic medical record (EMR) and care management platform. </li><li>Escalate complex clinical or social concerns to the RN or Utilization Management team. </li><li>Ensure compliance with HIPAA, state regulations, and organizational protocols. </li><li>Complete occasional field visits to SNFs or members’ homes in New Hampshire, with mileage reimbursement. <br><br></li></ul><strong>Qualifications<br><br></strong><ul><li>Active, unrestricted LPN license in the State of New Hampshire (required). </li><li>Minimum 2 years of nursing experience in long-term care, home health, rehabilitation, admissions, or care coordination. </li><li>Familiarity with New Hampshire Medicaid processes, coverage requirements, and facility workflows preferred. </li><li>Strong communication, organizational, and documentation skills. </li><li>Proficiency in EMR systems and care coordination technology. </li><li>Ability to work independently and manage a telephonic caseload in a remote setting. <br><br></li></ul><strong>Preferred Experience<br><br></strong><ul><li>Experience in managed care, home health, health plan case management, or utilization support. </li><li>Background in transitional care, discharge planning, or post-acute intake. <br><br></li></ul>Benefits Include<br><br><ul><li>$34/hour with mileage reimbursement for occasional field visits. </li><li>Temp-to-perm structure with opportunity for permanent employment. </li><li>Comprehensive health, dental, and vision coverage (upon permanent hire). </li><li>401(k) with company match (upon permanent hire). </li><li>Paid time off and holidays (upon permanent hire). </li><li>Continuing education and training support. </li><li>Remote-first work environment with organizational support.</li></ul>
