In this fully remote role, youll be a vital lifelineconnecting with patients, guiding them in managing their conditions, and making a real impact on their health outcomes.<br><br>What Youll Do<br><br>Connect & Care: Make monthly outreach calls to patients enrolled in our CCM program.<br><br>Personalized Care Plans: Collaborate with patients and providers to design and update individualized care strategies.<br><br>️ Early Intervention: Monitor symptoms, identify risks, and escalate care before complications arise.<br><br>Coordinate Care: Work seamlessly with primary care providers, specialists, and community resources.<br><br>Patient Education: Equip patients and caregivers with knowledge on disease management, medication adherence, and healthy habits.<br><br>Document with Precision: Keep accurate and timely patient records in our EHR system.<br><br>Stay Compliant: Follow CMS guidelines for CCM while promoting preventive care and lifestyle improvements.<br><br>What You Bring<br><br><ul><li> Valid RN license in the Philippines.</li><li> Minimum 1 year of clinical nursing experience (chronic care, case management, or primary care experience is a plus).</li><li> Strong knowledge of chronic conditions such as diabetes, COPD, hypertension, and heart failure.</li><li> Excellent communication, critical thinking, and organizational skills.</li><li> Comfortable with EHR systems and virtual care tools.</li><li> Able to work independently while thriving in a collaborative team setting.<br><br><br></li></ul>Bonus Points If You Have<br><br><ul><li> Familiarity with Medicare Chronic Care Management documentation and compliance.</li><li> Experience in motivational interviewing or other patient engagement methods.<br><br><br></li></ul>Ready to join us?<br><br>Send your resume to jobs@elevatecalls.com.
